Trinity Health Mid-Atlantic clinically integrated networks to unify into single collaborative partnership

Trinity Health Mid-Atlantic’s three separate clinically integrated networks – Quality Health Alliance, Mercy Accountable Care and Delaware Care Collaboration – will unify into a single network on Jan. 1.

Trinity Health Mid-Atlantic is a regional health system that includes St. Mary Medical Center in Langhorne.

The newly formed Trinity Health Mid-Atlantic Clinically Integrated Network (CIN), to be named Quality Health Alliance, will include a total of 500 primary care physicians, approximately 1,200 specialty physicians and close to 100,000 attributed lives, making it one of the largest clinically integrated networks in the Philadelphia tri-state area.

Trinity Health Mid-Atlantic CIN works to effectively coordinate treatment services for patients and provide access to alternatives to emergency departments and long skilled nursing facility stays when such care is needed. The network also emphasizes the importance of preventative screening, active chronic disease management, and the delivery of the highest quality of care to slow or prevent disease progression and the need for expensive hospital-based services. A multidisciplinary team works together to support Trinity Health Mid-Atlantic CIN’s integrated care coordination model.

Quality Health Alliance’s partners include Trinity Health Mid-Atlantic hospitals, Trinity Health Mid-Atlantic Medical Group physicians and independent affiliated community-based primary care and specialty physician practices. The network also maintains a robust care coordination team of nurses, social workers, behavioral health specialists, health coaches and other health care providers dedicated to providing comprehensive medical and non-medical management to its beneficiaries.

Additionally, Quality Health Alliance participates with virtually every major payor in the Greater Philadelphia area through value-based agreements, lowering costs for patients while providing participating physicians access to a wide range of insurance contracts.
